VIAL OF LIFE

Snohomish Fire District 4 has started a new service for our patients/customers. In conjunction with the King County Medic One Foundation, we will offer a "Vial of L.I.F.E." to our customers to aid the fire district crews in gathering vital emergency information. Please contact FF/PM Dave Alberts at 360-568-2141 for further information regarding this program.

 

WHAT DOES THE VIAL OF LIFE KIT CONTAIN?

  1. A completed Vial of Life form. (Answer the questions you want, don't answer questions you don't want)

  2. Providing a picture of yourself would be helpful.

  3. A photostat of your last EKG will greatly assist the emergency personnel.
    (Your doctor should gladly provide you with a copy)

  4. Place your Living Will or equivalent in your baggie, if you have one.

  5. Place any Do Not Resuscitate (DNR) documentation in the baggie, if you so choose.

  6. Place any other documentation you feel important in the baggie.
