Vial of L.I.F.E.
Snohomish Fire District 4 has a service for our patients/customers. In conjunction with the King County Medic One Foundation, we offer a Vial of L.I.F.E. (Lifesaving Information For Emergencies) to our customers to aid the Fire District crews in gathering vital emergency information. Come in to Headquarters Fire Station at 1525 Avenue D to get your free Vial of L.I.F.E.
What Does the Vial of L.I.F.E. Kit Contain?

A completed Vial of L.I.F.E. form. (Answer the questions you want, don't answer questions you don't want)

Providing a picture of yourself would be helpful.

A photocopy of your last EKG will greatly assist the emergency personnel. (Your doctor should gladly provide you with a copy)

Place your Living Will or equivalent in your baggie, if you have one.

Place any Do Not Resuscitate (DNR) documentation in the baggie, if you so choose.

Place any other documentation you feel important in the baggie.
